Questionnaires

Questionnaires enable you to gain critical information from requesters following the submission of the initial request, helping to understand any involved risks and how to properly triage their request. Unlike reviews, questionnaires do not block downstream processing by default—but you can configure the questionnaire to block downstream processing.

Create and Configure a Questionnaire

To create and configure questionnaires, follow the steps below:

  1. In the Questionnaires table, scroll to the bottom of the table and select the Create Row button, preference-table-create-row.png. The Create Row window displays.

  2. Configure by entering values for the relevant fields:

    1. Name (Required) - Enter a descriptive name for the questionnaire.

    2. Description - Enter a description of the questionnaire.

    3. Questionnaire Module (Required) - Select the module that contains the interfaces and workflow logic for the questionnaire. In most cases, this will be a custom module with your unique questionnaire logic, interfaces, and so on.

    4. Skip Conditions - Define the conditions that determine when the questionnaire is skipped in the process.

    5. Only Highlight as Needs Attention When - Define the conditions when the request is assigned the status of “Needs Attention.” This option is helpful for determining when a request is urgent.

    6. Block Downstream? - Select whether to prevent the request from continuing to downstream stages of the workflow until this questionnaire is completed.

  3. When finished, select Create. The questionnaire is created.

Always Add These Questionnaires

Select the questionnaires that are included in every request, unless relevant skip conditions are met.
